The Southwest Open School (SWOS) School Based Health Clinic (SBHC) offers SWOS students and all Montezuma County residents under age 21 limited primary medical services.
Southwest Open School (SWOS), School Based Health Center (SBHC) provides integrated primary and mental health care to students during school hours. Parental consent is obtained during school enrollment and as patients are seen in the SBHC. The SBHC is available to children and youth in our community under the age of twenty-one. The health center is under the medical sponsorship of Southwest Health System, Inc. and services provided include: comprehensive physical examinations, immunizations, treatment of acute illness, reproductive health care, and management of chronic conditions and behavioral health care. This could include behavioral/mental health education and consulting, crisis evaluation, and short-term therapies. The primary care provider and the behavioral health care provider work closely together for an integrated health care approach. Twice a year, the SBHC provides preventive dental care by a dental hygienist for dental screening and teeth cleaning, for SWOS students. Referrals are also made when appropriate to community and outside agencies.
The SBHC is under the medical sponsorship of Southwest Health System, Inc. and services provided include:

Regular Hours: 8:00-4:00 Monday-Thursday
August, January, May Hours: 8:00-4:00 Monday-Friday
